Hi,
Make a Template first. A template acts as a grid to place your elements. And then you can upload a image of a box after making one in MS paint, 
. Then say
In row 1 column 1 of template put text element1
In row 1 column 2 of template put box image
In row 1 column 3 of template put text element2
